Form Submission for the EXPO
Pre-approval is required for certain types of projects, specifically those involving Human Subjects, Vertebrate Animals, or Potentially Hazardous Biological Agents. The Scientific Review Committee will meet periodically to review the paperwork and determine whether the experiment may proceed, or whether changes are necessary.
All forms that need to be submitted can be sent electronically to firstname.lastname@example.org, or they may be sent via the USPS to the following address:

This should be used as a reference when determining that the student/project complies with requirements of the New Hampshire Science & Engineering Exposition and the International Science and Engineering Fair. For more detailed explanation of project topics, Scientific Review Committee (SRC), Institutional Review Board (IRB), and Consent Forms, refer to the Manual.

- If survey is used with human subjects, include copy with forms.
- DEA Controlled Substances, prescription drugs, alcohol and tobacco, firearms and explosives, hazardous chemicals and devices, and radiation are now a part of Hazardous Chemicals, Activities or Devices and do require a Qualified Scientist/Designated Supervisor. See Rules.
- All projects incorporating microorganisms, rDNA technologies, human or animal tissues, blood, or body fluids.
- Projects involving human subjects need to be sanctioned first by a local Institutional Review Board, and then sent in to the NHSEE Scientific Review Committee for approval to begin. For more info, see the Manual p. 5-9.
NOTE: Human Participants Form (4) is started by the student researcher(s) who complete the top part of the form, questions 1-4. The IRB then completes the middle part (either disapproval, or approval with questions 1-5), and then fills in all three signatures. The IRB should NEVER fill in the signatures and give a blank form to the students to complete it.
